COCO'S GYM Membership Agreement

I (members name), hereby indicate the desire to become a member of Coco's Gym, pursuant to the terms and conditions of this gym membership agreement.

Health Attestation

The Member attests that they are fully able to participate in an exercise regimen of their design or choosing without undue risk.

Indemnification

This gym membership serves as a liability release, pursuant to which the Member agrees to indemnify and hold Coco's Gym harmless against any and all claims of loss or damage without limitation.

Gym Rules & Participation

The Member hereby agrees to abide by all posted safety guidelines and regulations while using Coco's Gym facilities and equipment. Additionally, the Member agrees to dress and conduct themselves in a manner deemed appropriate for a fitness facility.​

The Member shall not consume drugs, alcohol , or tobacco products on Coco's Gym property. Coco's Gym reserves the right to revoke the Member’s access if these terms are violated.

Cancellation Policy

The member hereby agrees to give notice of cancellation 12 hours before a 1 on 1 session to receive a refund or make up session. Group class cancellations can be made within an hour of Class Schedule to receive a full refund. Cancellations made outside the stated hours will forfeit any refunds.
